If you require a same day appointment, please contact our office directly by calling 704-544-1300.  Reduce your time in our office by printing and completing New Patient Registration Forms (For Spanish, click here) before your appointment.  You may fax, e-mail the information or bring the forms to our Reception Area on or before your appointment date.  Our fax number is 704-752-2960 and our email address is seek@genesisent.com
